HEALTH CARDS

We require up-to-date OHIP card number with version code and expiry date to properly bill services provided to you. Failure to provide a valid health card may restrict your ability to receive services. If you are an existing rostered patient, please take the opportunity to provide us with your updated health care information.

APPOINTMENT INFO

There are two ways to book an appointment:

1.

Book in person or over the phone with one of our receptionists.

2.

Book an appointment online using the OAB platform.

POLICIES

The FFPG has policies that are relevant to patients of our clinic. New and existing patients of FFPG are encouraged to review them by clicking the policies below.

PATIENT RESPONSIBILITIES

  • Please treat all staff with respect. Rude or aggressive behaviour will not be tolerated and may result in the termination of the physician-patient relationship.
  • Please try to arrive 15 minutes before appointment time.
  • Please cancel appointments or no show fees will be charged.
  • Please bring your health card to every visit.
  • Please ensure your contact information (address, phone number, email address) are up to date in our system.

PRESCRIPTION RENEWALS

  • Plan in advance – order your medication before you need them.

  • Ask your pharmacy to send a prescription refill request to our office, it may take up to 4 weeks for the request to be considered.

  • You may be required to book an appointment if you have not seen your doctor in a while or if this is a medication that you do not normally receive.

  • Prescription renewals are not done after hours.

TRAVEL GRANTS

 As of December 1st, 2024 The Ministry has made updates to the Northern Health Travel Grant program. Referring Health Care Providers are no longer required to sign the Travel Grant, making the application process faster and easier. For more information you can click here.

During the Canada Post strike completed travel grants can be brought to Service Ontario.

Patients can pick up a blank travel grant located at the front entrance of the clinic or print one off of the Ministry of Health website

Take the travel grant to their out of town specialist appointment and have the specialist complete section 3

The patient will complete all their information and attach any receipts for lodging if applicable and mail it to the address provided on the travel grant
