Our current Doctors are not accepting any new patients at this time. However, you can click here to be added to our waitlist. If you are a prenatal patient or a child under 2 years of age who requires immunization, you can book an appointment.

If you need to leave a message for your Family Doctor you can call 807-274-3287 ext 249. Please note you may not be able to leave a message with your Family Doctor the day that you call as all of our Doctors take messages differently. If you do not get an answer you can leave your name and number and the message nurse will get back to you.

You can book an appointment with your Doctor by calling the appointment desk at 807-274-3287 ext 2 or online through Online Appointment Booking.

Ask your pharmacy to send a prescription refill request to our office, it may take up to 4 weeks for the request to be considered. You may be required to book an appointment if you have not seen your doctor in a while or if this is a medication that you do not normally receive.

If you have provided us with a valid email address, you will receive an email once the Specialist has received the referral along with an approximate wait time.

If you want to check on the status of your referral you can contact the Specialists office that you were referred to.

To obtain a copy of your medical records you would complete an Authorization for Record Release from the billing department. This would include the details of the records requested. The release form is then sent to your family Physician for approval and printing. Children over the age of 12 must sign their own release. You can click here to see the fee.
